New 2015 tax form 1095-C

The new 2015 tax form 1095-C is a requirement as part of the Affordable Care Act (ACA).  The purpose of the Form 1095-C is to provide information about the health coverage offered through an individual’s employer.

As an employer who offers health insurance, we are required to provide this notice to employees enrolled in our group health plan and to employees who qualified as a full-time employee (as defined by the Affordable Care Act) at least one month during the year.  As part of the contract, Equifax will print and mail the Form 1095-C to applicable employees.

Equifax will begin mailing the Form 1095-C to our employees beginning the last week of February to be delivered starting the first week of March.
