VACCINE ELIGIBILITY OPEN // PLEASE COMPLETE THE ONLINE FORM THIS MORNING

The online form to participate in a GHC onsite vaccination event has been extended to 9:30AM this morning (MARCH 24) following the announcement of increased eligibility.

Per Governor Brian Kemp’s announcement yesterday, all Georgians age 16 and over are now eligible to receive the COVID-19 vaccine. (Please note GHC’s vaccination event will be using the Moderna vaccine which is only eligible to those 18 and older.)

This is the final opportunity to sign up if you and/or any of your immediate family members are interested in taking part in an upcoming GHC onsite vaccination event. (If you have already completed this form, you do not need to do so again.)

College Update: COVID-19 Vaccines Now Available to GHC Employees 

Dear GHC Community,

In an effort to continue increasing our health and safety measures, GHC will offer an onsite vaccination event by partnering with Cartersville-based West End Pharmacy. (This event is not mandatory.)

GHC will work with West End Pharmacy to determine a specific day and time to administer the vaccine on our designated campus to eligible GHC employees and their immediate family members.

If you wish to participate in one of these vaccination events, please COMPLETE THIS FORM by providing your name, email address, and the GHC site where you would like to receive your initial vaccination. We will follow up soon with access to West End Pharmacy’s patient portal where you can complete your application for vaccination. 

Please note: While your privacy will be maintained under HIPAA law, GHC health science staff will assist in the process of the vaccination events, and nursing faculty will assist in providing vaccinations. While there are no guarantees on the specific vaccine, West End Pharmacy advises us it will likely be that by Moderna. There will be a second vaccination required one month after your first vaccination. You will be required to schedule your follow-up vaccination the day of your initial inoculation.
