Outside Activities and Conflict of Interest or Commitment

Previously there were two forms – Outside Activities and the Conflict of Interest or Commitment Disclosure – that employees were asked to complete when they participated in, or were wishing to participate in, activities or employment outside of the institution. To simplify the process and eliminate redundancy, the forms have been combined.

As discussed in the Annual Compliance training, the Conflict of Interest or Commitment Disclosure Statement is found on the HR website, Online Forms under the section Policy Related Forms, HR.103. Especially during a presidential election year, employees should be reminded that all outside activities and commitments, including political activity, should be reported for transparency. If you realize this applies to you, please complete the form and return it to your supervisor.
