USG Wellness Covid-19 Vaccine Reward

Did you receive the COVID-19 vaccine or plan to? You can earn a reward for your vaccination.

COVID-19 vaccination for You & Your Family, earn $10 (1x/year). *Earning period: Oct. 1, 2020 – Sept. 30, 2021.

To earn the reward all you have to do is receive the vaccine at a plan pharmacy, doctor’s office, or retail health clinic. If you already received the vaccination since Oct. 1, 2020 the reward may already be reflected on your rewards page. Confirmation of your vaccination is by a file feed with your healthcare plan, similar to how credit is earned for receiving your flu shot.

COVID-19 vaccination credit may take up to 8 weeks to show on your well-being credit page.

If it’s been more than 10 weeks since you were vaccinated and the reward is not showing on your Rewards page you can complete the Attestation Form to confirm you received the vaccination. Vaccinations completed at sites or events other than through your healthcare plan are also eligible for reward. Fill out the Attestation Form to receive credit.

For the COVID 19 Vaccine credit: When you log in to your USG Well-being account > Click Home > Click Rewards > Scroll down to “Receive the COVID-19 vaccine” > Click “Receive the COVID-19 vaccine.” > You will get a popup and click “TAKE ME THERE.” > Complete the COVID-19 Vaccine Attestation Form.
