FROM THE OFFICE OF THE PRESIDENT:

In an effort to continue increasing our health and safety measures, GHC will offer onsite vaccination events at two of our sites (Cartersville and Floyd) by partnering with Cartersville-based West End Pharmacy. (These events are not mandatory.)

GHC will work with West End Pharmacy to determine specific days and times to administer the vaccine on our designated campuses to eligible GHC employees and their immediate family members.

Current guidelines for those eligible to receive the vaccine can be found on the Georgia Department of Public Health website. (Since these guidelines continue to change with additional eligibility added, please check the DPH website often.)

If you wish to participate in one of these vaccination events, please COMPLETE THIS FORM by MONDAY (MARCH 22) by providing your name, email address, and the GHC site where you would like to receive your initial vaccination. We will follow up soon with days and times, as well as access to West End Pharmacy’s patient portal where you can complete your application for vaccination. 

Please note: While your privacy will be maintained under HIPAA law, GHC health science staff will assist in the process of the vaccination events, and nursing faculty will assist in providing vaccinations. While there are no guarantees on the specific vaccine, West End Pharmacy advises us it will likely be that by Moderna. There will be a second vaccination required one month after your first vaccination. You will be required to schedule your follow-up vaccination the day of your initial inoculation.
