As a reminder, Anthem and Northeast Georgia Health System have not reached an agreement on a new contract.

Therefore, Northeast Georgia Health System providers and facilities are out-of-network except for those employees or dependents that meet one of the following criteria and are approved to receive in-network level of benefits.  You are currently:

• In an active course of treatment for an acute medical condition, behavioral health condition, or serious chronic condition.
• Pregnant, regardless of trimester.
• Being treated for a terminal illness.

Northeast Georgia Health Systems covers service areas:

Barrow, Dawson, Franklin, Forsyth, Gwinnett, Habersham, Hart, Jackson, Lumpkin, Rabun, Stephens, and White.

To apply to receive in-network benefits (Continuation of Care), please complete the form available on the Anthem website.

Emergency medical care is always considered a covered benefit regardless of where you are treated.

Otherwise, for those employees and dependents who use providers or have services scheduled at facilities in Northeast Georgia Health System that do not meet the above criteria, please visit the Anthem website for more information about other options for care.
