Flu shots will be offered at Fall In-Service. They are FREE (and $25 in wellness credits!) for employees covered under a USG Medical Plan.
If employee does not have insurance the cost is $28.09 and will be collected the day of the clinic.
To receive your flu shot, please bring your medical ID card and form below.
