ACA Form 1095 Electronic Consent

You can now receive your medical coverage tax form, Form 1095, electronically.

The 1095 tax form shows that you and your dependents were offered or enrolled in medical coverage during the tax year. Proving that you’re adequately insured each year is mandatory and you’ll need this important form to complete your taxes.

You can still receive the Form 1095 by U.S. Postal Service mail if you prefer.  By choosing to have it available electronically, you can access the form immediately when it is ready, instead of waiting two to three weeks for it to arrive in the mail.

How to receive electronic notifications:

  • In order to sign-up to receive the Form 1095 electronic notification, log in to the OneUSG Connect – Benefits website and follow the instructions online.

When your request is processed, you’ll receive an electronic confirmation.

  • Please be sure to verify your contact information is correct in order to ensure your electronic notification will be sent to the appropriate email address/mobile number. You can do this by visiting the OneUSG Connect – Benefits website and following the instructions for signing up for electronic consent.
