Incorrect TINs or SSNs on 2015 Form 1095-C

Shared Services Center is sending out letters to employees that the IRS has identified as having possible incorrect SSNs or TIN (Taxpayer Identification Numbers).

The employees are being asked to review their 1095-C forms to verify name and social security numbers for themselves and covered dependents are correct.  No actions are needed from the employees if their records are accurate.  If USG records are not accurate, the letter requests that the employees contact the Shared Services Center to provide the change(s) that need to be made.
