CVS Formulary Changes 2017

Each year a clinical review of the CVS standard drug formulary is conducted in conjunction with FDA guidelines to review drugs within each drug class for clinical effectiveness. The goal is to ensure covered drugs possess the best treatment outcomes with minimal side effects.

Attached you will find the 2017 Formulary Drug and Exclusions Lists, which has also been posted to the USG pharmacy benefits webpage as a resource

USG members who may be impacted by the drug exclusions list will begin receiving notification from CVS Caremark this week, providing them with additional instructions on actions they will need to take with their primary care provider.
