UNUM – Critical Illness

Critical Illness Insurance:

Unum’s Critical illness insurance can pay a lump sum benefit at the diagnosis of a covered illness. You choose the level of coverage — either $5,000 or $10,000 for employees — and you can use the money any way you see fit. Spouse coverage of $5,000 is available. Children are automatically covered at 25% of the employee amount. It is designed to help you offset the financial effects of a catastrophic illness with a lump sum benefit when you are diagnosed with a covered critical illness.

Enrollment is available: Monday, November 2nd – Friday, November 13, 2015

  • a guarantee issue of $10,000 for employees and $5,000 for spouses- which means NO medical questions asked.
  • an annual wellness benefit of $50 per insured.
  • no pre-existing condition exclusions.

Employees hired before 11/01/2014, you have already been given the opportunity to enroll in this coverage under a guarantee issue basis. During this annual enrollment period if you decide that you wish to enroll in one or both of these plans you will have to answer the medical question associated with the benefits. This can be done during open enrollment online at www.HighLowOrNo.com by entering your log in information below. If you are currently enrolled, you do not need to enroll again. Existing coverage will continue as is.
