In our continued effort to focus on wellness, Rite-Aid will be on our campuses to provide flu shots for employees and family members. The flu shots are covered at a $0 copay for the BCBSGa pharmacy business with the BOR. For those not covered through the BOR plan, the cost will be $25.
In order for Rite-Aid to process the claims you will need to bring your insurance and pharmacy card with you. Attached is a consent form. Be sure to print and complete the form prior to coming to your appointment.
Family members are welcome to participate. Children under 13 years of age require a prescription from their doctor to be vaccinated.
There will be a voluntary sign-in sheet at each location. Each employee who receives a vaccination and signs the voluntary sign-in sheet will be awarded 35 points in the GHC Wellness Incentive Program.
