On Monday, December 16, 2013, Minnesota Life shipped the Evidence of Insurability (EOI) packets to the employees and spouses who are pending EOI.

Employees with an email address on file will receive an email from Minnesota Life. Spouses and employees with no email address will receive a packet in the mail at their mailing address on file.
