Please note:
- Employee ID number is your OneUSG number not your BANNER ID/GHC ID. You can locate your Employee ID number on your paycheck stub.
- Please include your Student Id Number.
- Please include both your signature and your supervisor’s signature.
- TAP is only available to full-time benefits-eligible employees who have successfully completed at least six months of employment in a benefits-eligible position, as of the date of the TAP application deadline for the desired academic semester. The exception is for part-time nursing faculty seeking a graduate degree in nursing.
