To encourage vaccination, USG is offering an increased Well-being credit to all employees enrolled in a USG healthcare plan and their eligible spouses who get vaccinated.
Vaccine credit may take up to 8 weeks to show on your well-being credit page when
completed at a plan pharmacy, doctor’s office, or retail clinic. Complete the Attestation Form if it’s been
more than 8 weeks, or you received the vaccination from another provider.
